Of course telehealth must be user-friendly, secure, reliable and tested, but to be at its most effective it also needs to be carefully embedded into care pathways. That's the Tunstall difference. Our technology is supported by the very best in project management and clinical engagement, including extensive hands-on experience of delivering successful, integrated telehealth services built around the patient's needs.
Tunstall's telehealth managed service offering includes:
Tunstall telehealth partners:
Tunstall's ICP telehealth solutions help to support people with long-term conditions by enabling them to monitor their vital signs and symptoms daily in their own homes, with the reassurance that if their readings go beyond the limits set for them, a clinician will be informed. Telehealth improves the patient's understanding of their condition, which in turn increases their confidence and empowers them to manage their own health with the support of their health and care team.
Tunstall's telehealth solutions are tailored around the needs of the individual patient, and a number of medical grade peripherals are available to support and manage long-term conditions.
The data collected via the myclinic telehealth hub is securely transmitted to the triage monitoring centre for review by a clinician. Abnormal readings for the patient are flagged for follow up and intervention such as clinical coaching, nursing visit or GP appointment. The collected data may also be passed on to the patient’s doctor for further review or intervention.
The ICP solution allows the patients’ care team including community nurses, GPs and specialists to closely monitor the patient’s condition and intervene before it reaches crisis level and results in hospitalisation. ICP also offers video conferencing at the press of a button, allowing patients to maintain face to face contact with their clinician regardless of their location or medical state.
Tunstall's patient management system ICP triage manager provides clinicians with ongoing access to patient health information and trend data. ICP triage manager can integrate with other patient management systems and enhances clinician workflow and standardisation, allowing patients to be prioritised on a needs basis.
