ICP supports remote monitoring of patient vital signs and health conditions from their own home - case studies have shown examples where telemonitoring has improved the quality of life and clinical outcomes for people with long term conditions. It also helps people understand and manage their own health, where in some instances it can lead to a reduction in hospital visits.
With access to both real-time and historical vital sign and health interview data, clinicians, patients and caregivers can monitor trends and changes in their condition, in turn informing clinical decision making and lifestyle changes.
